How has participating in Minds Matter changed you?

The scholarship shall be administered by a committee partially appointed by the Board of Directors, which shall be provided for by resolutions of the directors. Decisions made by the committee are based on criteria set out in the general guideline section of the application.

I agree to comply with the guidelines of the scholarship attached to this application and understand that this scholarship is only to be used in accordance with those guidelines and is subject to forfeiture based upon the conditions of those guidelines. Student accepted must provide confirmation of college enrollment.
